What causes MS fatigue?

Fatigue is one of the most common and most disruptive symptoms of multiple sclerosis. If you live with MS, you may recognise it: a sudden, overwhelming exhaustion that can arrive without warning, often has little to do with how much you have done, and is not always helped by rest.

What makes MS fatigue different is that it usually has more than one cause. It can be primary, driven directly by the disease process in the brain and spinal cord, or secondary, brought on by other effects of MS such as poor sleep, pain or medication. Understanding which factors are at play for you is the first step towards managing it.

Is MS fatigue different from normal tiredness?

Yes. Ordinary tiredness builds up after exertion and improves with rest or sleep. MS fatigue tends to:

  • Come on quickly and feel overwhelming
  • Be out of proportion to what you have done
  • Persist or even worsen after resting
  • Worsen in heat, with infection, or after sustained concentration
  • Affect the whole body as well as mental (“cognitive”) stamina

This is why MS fatigue is sometimes described as “brain fatigue”: it is rooted in the central nervous system rather than simply in tired muscles.

What causes MS fatigue directly (primary fatigue)

MS damages the myelin sheath that protects nerve fibres, and the nerves themselves. This slows and disrupts the electrical signals that travel between your brain and body. To compensate, the nervous system has to work harder and use more energy simply to send the same messages, which is genuinely exhausting.

Research suggests several direct contributors:

  • Nerve conduction changes from demyelination and axonal damage
  • Inflammatory activity in the central nervous system
  • Lesions in areas of the brain involved in attention, arousal and energy regulation

In other words, the effort your brain expends to keep signals moving can itself be a major source of fatigue.

What causes MS fatigue indirectly (secondary fatigue)

Many people with MS also experience fatigue that is driven by other MS-related factors. These are often the most treatable:

  • Poor sleep: nocturia (needing the toilet at night), muscle spasms, pain and anxiety all fragment sleep.
  • Heat sensitivity: a rise in body temperature (Uhthoff’s phenomenon) can temporarily slow nerve conduction and intensify fatigue.
  • Medication side effects: some disease-modifying therapies and other medicines list fatigue or drowsiness as a side effect.
  • Depression and anxiety, common in MS and closely linked with fatigue, each worsening the other.
  • Pain and spasticity: constant muscle tension and nerve pain drain energy.
  • Reduced physical fitness: staying active can be hard during a relapse or flare, and deconditioning lowers stamina.
  • Other conditions: anaemia, an underactive thyroid, or vitamin deficiencies can all add to fatigue and are worth checking.

Because several of these can stack on top of one another, small improvements in any one area can make a noticeable difference.

Cognitive fatigue

Many people with MS notice that thinking hard, concentrating in a meeting, processing information, or simply holding several things in mind, drains them as much as physical activity. This “cognitive fatigue” is a recognised part of MS and is thought to relate to the extra effort the brain uses to perform tasks affected by lesions. It is real, and pacing mental tasks is just as important as pacing physical ones.

When to speak to your healthcare team

Fatigue that is new, worsening, or getting in the way of daily life is worth raising with your MS nurse or neurologist. They can check for reversible causes (such as anaemia or thyroid problems), review your medications, and discuss approaches like energy management, exercise programmes, sleep support or, in some cases, medication.
